payback_days        = ceil(plan_cost_month ÷ (recoverable_month ÷ 30.4))

How the estimate works

The calculator multiplies your calls per day by the days you're open and the share of calls you miss. Of those missed calls, it counts the ones that were people wanting an appointment, and of those, the callers who don't call back and probably book somewhere else. That number, times your average visit value, is the estimate of visits not booked each month.

  • Share of calls missed starts at 25%, the low end of a published range for small and medium clinics. Your own call log gives you the real figure.
  • Share of missed calls that are bookings starts at 40%. Change it to match your clinic's call mix.
  • Average visit value starts at $185. Use the average from your practice software if you know it.
  • Share of missed callers who don't call back starts at 60%. This is our assumption, not a statistic: no published source measures it. Change it if you know your own figure.
  • Days open per month starts at 22, and share of answered calls Tilly can turn into a booking or task at 70%.

What the estimate leaves out

The estimate counts visits, not clients. It leaves out the lifetime value of a client who goes elsewhere, the staff time spent on callbacks and voicemail, and the pressure on a front desk that can't get to the phone.
